Job Completion Form: What To Include + Template For Field Teams

Last Updated: October 27, 2026
job completition report

Picture a plumber on your team who replaces a water heater for a customer, then drives to the next address on his list.

Two weeks later, the customer disputes the invoice, saying the old unit was never taken away.

You can’t prove the job was done because there’s no signature, no photo and nothing on file except the technician’s words.

About 4 in 5 small businesses face challenges related to customer payments, according to the Federal Reserve’s Small Business Credit Survey.

Some of that money is stuck because the documents can’t answer a simple question: was the job actually finished?

In this guide, we’ll cover:

  • What a job completion form should include
  • A job completion report template you can customize for your company
  • How to fill out the form step by step
  • How a job completion form differs from a work order and a field service report
  • What changes when you digitize your forms

What Should A Job Completion Form Include

Every field on a job completion form is important for your team.

The person sending the invoice checks out the details, the manager reviewing the job confirms the scope was completed, and a technician returning for a warranty call looks up what was installed and when.

Job & Customer Details

Include the customer’s name, site address, job or work order number, and the technician or crew assigned.

The work order number will help you:

  • Link the completion record to the original request
  • Find the full job history when a customer calls with questions.

Work Performed

Describe what was done in detail so anyone can understand it without the need for extra calls.

Instead of just writing “WH replaced,” use “Replaced 50-gallon gas water heater and installed a new thermal expansion tank.”

Instead of “Fixed door,” write “Replaced worn door closer on rear fire exit, tested self-closing and latching.”

Materials & Equipment Used

List the parts you’ve used along with the model and serial numbers. This is important if the equipment fails under warranty, since manufacturers typically ask for the serial number before approving a claim.

Time On Site

Record arrival time, departure time and total labor hours. These hours go straight onto the invoice, ensuring you’re billing the right amount.

Photos

A photo of the finished site is often the fastest way to settle a “you left a mess” complaint.

Capture the installed equipment, its data plate, key connections, and how the site looked after the job was completed.

On construction and repair jobs, photos also show the work area was cleared of debris, as the Occupational Safety and Health Administration (OSHA)’s standard requires.

Tests & Checks Completed

Record any test that proves the equipment is functioning. This can include a pressure test or system start-up, along with the actual result or reading.

Follow-Up Details

List any open issues, including a back-ordered part, a second visit or a problem spotted outside the job scope.

Assign each item to a team member and include a due date by which the new job must be completed.

Customer Sign-Off

Get the customer’s printed name, signature and date confirming the work was completed as described.

They can document their concerns in a comments field on the spot before the invoice is sent out.

How To Fill Out A Job Completion Form Step By Step

The best time to fill out your work completion form is before you leave the site, while you can still check every detail.

Check the Work Against The Work Order

Pull up the original work order and go through each line carefully. If something wasn’t done, add it to the follow-up section, so the form reflects the actual situation.

Test Before You Record

Run the system, check if everything is working and take readings.

Record Materials Before You Pack Up

Count what you took from the truck and used for the job while the empty boxes are still on site.

You may easily forget something if you rely on your memory or fill the job completion form at the end of the week.

Take Photos

Once the work is finished and the area is cleaned up, take your final set of photos.

Include:

  • A wide shot of the finished installation
  • A close-up of the equipment data plate where one can see the model and serial numbers
  • The work area as you’re leaving it

If you took a “before” photo, shoot the “after” from the same angle. This will help the person reviewing the job see the difference clearly.

Walk The Customer Through The Work

Show the customer what was done, explain if follow-up is needed and answer any questions they might have.

A signature carries more weight when the customer has seen the finished job.

Sign & Collect The Customer’s Signature

Add your signature and ask the customer to sign the job completion report.

If the customer isn’t there, write the name of the person you spoke with and send the form for sign-off the same day.

Job Completion Report vs. Work Order vs. Field Service Report

It’s easy to mix up these three documents because they share many of the same fields, such as customer details and technician names.

What separates them is when each one is used and what it shows.

  • A work order comes first. It authorizes the job, describes the scope and assigns it to a technician or crew.
  • A field service report documents the work done during a visit: what the technician found, diagnosed and did on site. If a project requires several visits, a technician may file one report for each visit.
  • A job completion report confirms the work is finished. It’s filled out at the end of a service call or project, lists anything still open and captures the customer’s acceptance.

Many businesses use these terms interchangeably, and on a one-visit repair a single form often does both jobs.

Create Digital Job Completion Forms With doForms

If your crews still fill out job completion reports on paper, you know that they can keep them in the truck for days before reaching the office.

It’s also possible that photos and signatures can end up in different places.

doForms is a mobile forms solution that lets your team fill out, sign and submit a job completion form on a phone, tablet or laptop before leaving the site.

All you have to do is build the form once, using a ready template or building your own with the drag-and-drop mobile form builder.

Add the fields your jobs need, from the scope checklist to the customer sign-off, without any coding.

Once your job completion form is ready, your team can fill it out, attach photos, collect electronic signatures and record GPS location. Every entry is timestamped automatically.

When a form is submitted, it can be routed to your accounting team, which can send the invoice to the customer the same day.

doForms can also cover other processes related to the work completion form, such as:

  • Dispatching jobs
  • Scheduling them on a calendar
  • Connecting completed forms to the business systems you already use

Three plans let you start by digitizing your paper form and add workflow automation and integrations as your team grows.

New customers can try doForms free for 30 days.
